II. PAYMENT ELECTION
If you are a member of the Class and are entitled to receive a share of the distribution of the Net Settlement Fund, and you have provided your name and updated address, along with your mail id, in Section I. above, a check will be sent to you at that address.

In case you prefer to receive an electronic payment instead of a check, please fill out the information in Section I. above, including your email address, and then choose your payment option in this Section II. below.

- I choose to receive my distribution payment from the Net Settlement Fund via electronic payment, which will be sent to me by email to the email address identified in Section I above; (At the time of distribution, those receiving electronic payments will be able to select between electronic Mastercard, Venmo, or PayPal.)
- I choose to receive my distribution payment from the Net Settlement Fund via a paper check, which will be mailed to me at the address listed in Section I above.
If a form of payment is not selected above in Section II, the default payment form will be a paper check.
